Estrogen is a hormone which among other actions, regulates the turnover (formation and destruction) of bone. Decreases in estrogen levels that are seen after menopause or after removal of the ovaries, lead to a loss of bone density and weakened bones, a condition called osteoporosis. Evista decreases bone turnover and increases bone density although not to the same extent as estrogen itself. This makes bones stronger and prevents fractures in women with osteoporosis. Evista is called a "selective estrogen receptor modulator" since it has effects like estrogen on some tissues but inhibits the effects of estrogen on other tissues. Evista decreases low density lipoprotein (LDL or "bad") cholesterol in the blood; however, unlike estrogen, evista does not increase high density lipoprotein (HDL or "good") cholesterol.
Raloxifene is an oral selective estrogen receptor modulator (SERM)
which is used in the prevention of osteoporosis in postmenopausal
women. It was announced on April 17, 2006, that raloxifene is as
effective as tamoxifen in reducing the incidence of breast cancer
in certain high risk groups of females, though with a reduced risk
of thromboembolic events and cataracts in patients taking raloxifene
versus those taking tamoxifen. On September 14, 2007, the U.S. Food
and Drug Administration announced approval of raloxifene for reducing
the risk of invasive breast cancer in postmenopausal women with
osteoporosis and in postmenopausal women at high risk for invasive
breast cancer.The recommended dosage is one 60-mg Evista tablet daily, which may be administered any time of day without regard to meals.
Evista (Raloxifene) may cause side effects. Tell your doctor if
any of these symptoms are severe or do not go away:
hot flashes (more common in the first 6 months of Raloxifene therapy)
leg cramps
swelling of the hands, feet, ankles, or lower legs
Some side effects can be serious. The following symptoms are uncommon,
but if you experience any of them, call your doctor immediately:
sudden chest pain or chest heaviness
difficulty breathing or coughing up blood
pain, swelling, or warmth in the calves, legs, hands, or feet
sudden change in your vision such as vision loss or blurring
In an 8-week study of 63 postmenopausal women, a dose of raloxifene
HCl 600 mg/day was safely tolerated. In clinical trials, no raloxifene
overdose has been reported. In postmarketing spontaneous reports,
raloxifene overdose has been reported very rarely (less than 1 out
of 10,000 [< 0.01%] patients treated). The highest overdose has
been approximately 1.5 grams. No fatalities associated with raloxifene
overdose have been reported. Adverse reactions were reported in
approximately half of the adults who took 180 mg raloxifene and
included leg cramps and dizziness.
Before having any surgery, tell your doctor that you are taking Evista. Treatment with Evista may need to be stopped temporarily if you require an extended period of bed rest.
